GP Care also has a number of further worked-up service models that are ready and available to NHS commissioners as set out below, all of which are delivered to high standards of clinical care, from community settings providing for easy patient access, and at significant discounts to National Tariff.
- End of Life Plan Service - A patient record IT system that provides real time access to End of Life Plans for NHS patients that helps ensure that patients are treated according to their wishes and in line with the clinical advice that they have received. This will reduce hospital admission rates where patients wish to remain at home and ensure that there is no confusion as regards the type of care required at a particularly difficult time for patients and their families;
- Care Plan Service - A patient record IT system that provides real time access to Care Plans for NHS patients that helps clinicians (including GPs, hospital doctors, nurses etc) provide care that is consistent with the patient's overall Care Plan. This will ensure that there is no confusion amongst clinicians as regards the strategy for caring for individual patients and will ultimately speed up the process of patient recovery and lead to earlier discharge from hospital;
- Skin Cancer Service (Basal Cell Carcinomas excision) - A community based BCC excision service, provided by accredited GPs with specialist skills in dermatology/surgery from a network of clinic locations. This service will provide fast and convenient access to patients who are currently enduring long waits for hospital appointments; it is provided at a significant discount to NHS National Tariff and will deliver significant savings to commissioners;
- Gynaecology Service - provision of gynaecology diagnostic and assessment services from a network of community settings. This will deliver a service that is convenient for patients, giving fast access to clinics which are held in convenient community locations, and deliver significant cost savings to NHS commissioners on account of the service being delivered at significantly below NHS National Tariffs;
- ENT Service - provision of ENT diagnostic and assessment services from a network of community settings. This will deliver a service that is convenient for patients, giving fast access to clinics which are held in convenient community locations, and deliver significant cost savings to NHS commissioners on account of the service being delivered at significantly below NHS National Tariffs;
- Palivizumab Service - provision of a community based Palivizumab vaccination service, including home delivery, to protect 'at risk' babies from Respiratory Syncitial Virus. This service will achieve high completion rates which have generally not been realised via hospital delivered programmes, will be delivered at multiple clinic locations which are more accessible and convenient for patients, and will be delivered to NHS commissioners at below NHS National Tariff.
